I thinks it's some kind of Ford. Is it a converted off-roader?

Looks like a fairly normal dual-cab F350 (or whatever - some bloaty
'merkin pick-'em-up) to me.

It's not a converted anything. It's a pick-up truck.

Probably a pre-2000 Ford F350 with suspension lifts from the look of it.
Usually the wheels don't have that much clearance from the wheel arches.
It looks like one of these:
