change of name deed, birth certificate , prove of where you live, and
details of your old drivers licence (address, name it was under,
date of birth etc) get a form from the post office for getting a
driving license, this should have the details on it.

Did you actually read the OP? The licence is lost...

It's only 'free' if you're still in possession...

My apologies. I missed that.

I can't see why though - you have to provide the same documents and forms
whether you have the original licence or not, so it's presumably the same
amount of work for them...
