I think it was DW (Dim Wit) that said it could have been the lugs or lug
nuts, but it might have been from an earlier stop. Something like if
the lugs were loose earlier they might have had their threads damaged by
a loose wheel. Once damaged the threads might not have held on
subsequent stops. Then the wheel would start vibrating and the weakened
lugs and / or lug nuts would fail.
I'm not speculating one way or the other. Just trying to recall his
explanation.
